Live Better Louisiana Wellness Program

Live Better Louisiana is an Office of Group Benefits (OGB) program designed to help Blue Cross plan members* take charge of their health. Through helpful tools and resources, members can track their wellness, better understand their risk factors, and make informed choices to stay healthier—all while earning a $10 monthly discount on their health care premiums. The program is sponsored by Blue Cross and Blue Shield of Louisiana at no additional cost.

Live Better Louisiana serves as your roadmap to better health. It emphasizes prevention whenever possible and provides support for managing conditions if they arise. Participants can access personalized wellness insights, engage in healthy activities, and build lasting habits that support long-term well-being.

* To be eligible, members must be enrolled in one of the following BCBS Health Plans: Pelican HRA1000, Pelican HSA775, Magnolia Local, Magnolia Local Plus, Magnolia Open Access. Members enrolled in the Blue Cross Medicare Advantage plan, Blue Advantage, are not eligible to participate.

New: Wellness Exams Facilitated by Ochsner Health

Ochsner Health is now facilitating wellness exams for eligible members and assisting with the processing of Primary Care Provider (PCP) forms. This added support helps streamline participation in the program and ensures members can easily complete required steps to qualify for their premium discount.

2027 Ochsner Health Medical Exemption Form

This form is completed by the physician. If you did not qualify for the wellness incentive or are unable to participate in the onsite preventive checkups because you were pregnant at the time of the checkups, as an alternative you may work with your physician to develop a plan to maintain or improve your health.

2027 Ochsner Health Primary Care Provider Form

If you were not able to receive a Checkup this year, you may have your Primary Care Provider report lab and biometric values to receive credit toward the Office of Group Benefits wellness incentive being offered.
